Physical Requirements
All activities involve high levels of concentration and mental attention. Requires handling of light-weight objects up to 10 pounds, or some standing or walking.
Shift
M-F, 8:00am-4:30pm, occasional weekends and holidays.
Job Summary
The Patient Care Facilitator is responsible for the coordination of patient care, promoting fiscal responsibility and optimizing patient and family outcomes. This person collaborates with multidisciplinary groups in the health care system to achieve these outcomes for the hospital inpatients. Goals are achieved through coordinating discharge planning and problem-solving issues in ways to facilitate movement of patients through the system. The position reports to the Manager of Patient and Family Services and works directly with the medical staff of the designated medical or surgical services at Good Samaritan Hospital.
Skills / Knowledge / Abilities
Coordinates and collaborates with multidisciplinary groups in the health care system. Uses knowledge of nursing process, clinical concepts and coordinates care according to established practice guidelines. Ability to work with groups in assisting with process improvement initiatives. Clinical expert in the area. Excellent communication skills and goal orientation.

Preferred Education/Experience
BSN preferred with 3 years of experience-RN graduate of an accredited school of nursing, KY License Eligible.
